Please complete the Accessibility Accommodation Request form to the best of your ability. To schedule an appointment with the Disability Support Services (DSS) Office to discuss possible accommodations, you must be an admitted student and provide the required documentation before your appointment. Accepted documentation is listed below:

- Individualized Education Plan

- 504 Plan

- A psychological evaluation

Providing sufficient documentation is the first step in the process to request accommodations. If you submit the request form without documentation, we will not be able to process your request and schedule a meeting. 

Schoolcraft College reserves the right to request additional or updated documentation. 

Documentation Submission

All information provided will remain confidential and be submitted directly to the DSS office.

Within three business days, the DSS office will contact you via your Schoolcraft email to confirm whether your documentation is sufficient and schedule an appointment, or request additional information, if needed.
